THE AUSTRALIAN CHILDHOOD FOUNDATION’S CHILDHOOD HERO PHOTO COMPETITION 2007
Step 1: Download Consent From
Visit the http://childhoodhero.fujicolor.com.au (“the Web Site”) and download a copy of the Consent Form. If you plan to enter by mail download a copy of the Registration Form at the same time.
Step 2: Obtain permission from parent(s) or guardian(s)
- Under 18s: If you are under 18 years of age at the time of entry, seek your parent’(s) or guardian’(s) consent to enter the competition by having them tick the appropriate box and sign in the place provided on the Consent Form. If your parent(s) or guardian(s) do not agree with you entering the competition then you cannot enter.
In addition to the above, request whether they will consent for your Childhood Hero Photograph to be used for the advertising purposes disclosed in the Consent Form by ticking the appropriate box. Further, you must also indicate whether your Childhood Hero Photograph can be used for the advertising purposes disclosed in the Consent Form by ticking the appropriate box and signing in the correct place. If your parent(s) or guardian(s) or Childhood Hero do not agree to the use of your Childhood Hero Photograph for the advertising purposes outlined in the Consent Form then it will not be used for such purposes. However, your Childhood Photograph can still be submitted for the purposes of entry into the competition, provided your parent(s) or guardian(s) agree to this, as well as your Childhood Hero.
- If you are 18: If you are 18 years of age at the time of entry, you are NOT required to seek your parent(s) or guardian(s) consent to enter. In addition, you must indicate whether your Childhood Hero Photograph can be used for the advertising purposes disclosed in the Consent Form by ticking the appropriate boxes and signing in the correct place. If you or your Childhood Hero do not agree to the use of your Childhood Hero Photograph for the purposes outlined in the Consent Form then it will not be used for such purposes. However, your Childhood Photograph can still be submitted for the purposes of the competition, provided that the Childhood Hero agrees to this.
Step 3: Obtain permission from Childhood Hero
Obtain the permission of the person who you wish to submit as your Childhood Hero to take his or her photograph for the purpose of the competition. Childhood Heroes must be aged 18 years or older. Only one Childhood Hero can be shown in each Childhood Hero Photograph submitted. Show your Childhood Hero a copy of the Consent Form and ask whether they will consent for his or her photograph to be submitted for the purposes of the competition. Further, ask him or her to complete the appropriate places on the Consent Form, including whether they consent for the use of his or her photograph for the advertising purposes outlined in the Consent Form by ticking the appropriate boxes and signing in the correct place.
Step 4: Take your Childhood Hero Photograph
Once the Consent Form has been completed by your parent(s) or guardian(s) and your Childhood Hero, take the photograph of your Childhood Hero.
Step 5: Enter the competition either online or via mail *
(A) Mail: To enter by mail:
- Print your film or digital photograph of the Childhood Hero.
- Complete the Registration Form which can be downloaded from http://childhoodhero.fujicolor.com.au.
- Attach the completed Consent Form and Registration Form to your Childhood Hero Photograph and enclose in an envelope and mail to: Childhood Hero Photo Competition 2007, PO Box 525, Ringwood Vic 3134.
(B) Online: To enter online:
- Visit the website http://childhoodhero.fujicolor.com.au and follow the prompts through to the competition entry page and upload your Childhood Hero Photograph. In addition, the fully completed Consent Form must be mailed to: Childhood Hero Photo Competition 2007, PO Box 525, Ringwood Vic 3134, or faxed to (03) 9879 7388.
*All entries, including Consent Forms, must be received by 5pm AEST on 20 June 2007.
SCHOOL COMPONENT
An authorised representative of an eligible school must complete the Fundraising Form and fax it to (03) 9879 7388 by 5pm AEST on 20 June 2007. The original version must be posted with the School’s cheque made out to Australian Childhood Foundation:

CONDITIONS OF ENTRY
- Information on how to enter and prizes forms part of these Conditions of Entry. Entry into this competition is deemed to be acceptance of these Conditions of Entry.
CHILDHOOD HERO COMPONENT
- Entry is open to residents of Australia between the ages of 4 and 18 inclusive as at the time of entry.
- Employees, management and directors of the Promoter, FUJIFILM Australia Pty Ltd (ABN 80 000 064 433), and their agencies associated with this promotion (“Ineligible Individuals”) and the immediate families of Ineligible Individuals are not eligible to enter this competition.
- Photographs will not be returned to any entrant. Unless indicated otherwise by the entrant (and parent(s) or guardian(s), where applicable) on the Consent Form, the entrant agrees to assign all of their rights, title and interest (including copyright) in and to their Childhood Hero Photographs to the Promoter. The entrant (and their parent(s) or guardian(s), where applicable) warrants to the Promoter that each photograph submitted is an original artistic work of the entrant which does not infringe the rights of any third party. The entrant (and their parent(s) or guardian(s), where applicable) warrants that they have obtained the consent of the Childhood Hero to submit the entrant’s Childhood Hero Photograph. The entrant (and their Guardian, where applicable) indemnifies the Promoter against all claims, costs and liability that the Promoter may incur in relation to a breach of the warranty set out in this condition 4.
- Promotion commences 16 April 2007. Entries close at 5pm AEST on 20 June 2007. This is a game of skill and chance plays no part in determining the winner. Each photograph will be individually judged based on originality, creativity and inspiration within each category. Judges' decision is final and no correspondence will be entered into.
- Multiple entries accepted but each entry must be submitted separately and must be accompanied by its own completed Registration Form and Consent Form.
- Judging will commence on 23 June 2007. Each entry will be sorted into either Pre school/Prep – Grade 3; Grade 4 – 6; Year 7 – 10 and Year 11 - 12 according to the grade of the child that appears on the Registration Form as at 20 June 2007. The winning entries will be announced on 28 June 2007.
- The best entry from each age category, as determined by the judges’, will win a Fujifilm S6500fd digital camera plus a media card valued at $600 RRP.
- The overall winning entry, being the best entry judged from the four age category winning entries, as determined by the judges, will win, in addition to the category prize of a Fujifilm S6500fd digital camera as specified in clause 8, a visit for two people to the set of Channel Ten’s TV show, Neighbours. The visit to the set will take place on a date to be determined by the Promoter in consultation with Channel Ten. At least one parent or guardian must accompany the winner (if under 18 years of age) while attending the set. All costs associated with attending the set including transport to and from the set are not included.
- The first 100 valid online entries received will each win a Fujifilm QuickSnap disposable camera valued at $18.00 RRP. Total value of these prizes is $1,800.00 RRP.
SCHOOL COMPONENT
- Promotion commences 16 April 2007. Entries close at 5pm AEST on 20 June 2007. This is a game of skill and chance plays no part in determining the winner.
- Each participating school will be ranked according to the total funds submitted to the Promoter divided by the number of their students who participated in the Childhood Hero component of the promotion to determine the School Donation Score. The school with the highest School Donation Score will be declared the winner. If there are two or more schools with the highest School Donation Score, then the Promoter will assess the total amount of funds donated to rank these tied schools with school making the larger donation ranked higher. If after this process there is no clear winner, the Promoter will request that the authorized representatives on behalf of the schools that are tied for first place, submit an answer to a game of skill question within a specific timeframe. These entries will be judged on creative and literary merit with the best entry being declared the winner.
- The winning school, as determined by the Promoter, will have Neighbour’s stars Stefan Dennis and Pippa Black attend the winning school, on a date to be determined by the Promoter. The winning school will also receive the following: one (1) Fujifilm S5 Pro Digital SLR camera plus a printer and lens valued at $5,000.00 RRP.
GENERAL
- The Promoter reserves the right to verify all entries and entrant’s (including their name and age) and disqualify any entrant for tampering with the entry process or for submitting an entry which is not in accordance with these conditions of entry, including not sending in a properly completed Consent Form.
- Prizes, or any unused portion of a prize, are not transferable or exchangeable and cannot be taken as cash.
- Selected photographs that have been entered may in the absolute discretion of the Promoter be published on the Promoter’s website during the competition, subject to the relevant consent being provided. The appearance of any photographs on the Promoter’s website does not indicate that any of these photographs will necessarily be winning photographs.
- Winners will be notified by phone and confirmed by mail.
- If the winner is under the age of 18 years, the prize will be awarded to the winner's parent or guardian.
- All online entries are deemed to be received at the time of receipt into the promotional database NOT time of transmission by the entrant.
- The Promoter accepts no responsibility for any loss incurred in the event that the celebrity visit is cancelled for any reason beyond the control of the Promoter.
- If this promotion is interfered with in any way or is not capable of being conducted as reasonably anticipated due to any reason beyond the reasonable control of the Promoter, the Promoter reserves the right, in its sole discretion, to the fullest extent permitted by law to (a) disqualify any entrant; and (b) to modify, suspend, terminate or cancel the promotion, as appropriate.
- Except for any liability that cannot be excluded by law, the Promoter (including its officers, employees and agents) excludes all liability (including negligence), for any personal injury; or any loss or damage (including loss of opportunity); whether direct, indirect, special or consequential, arising in any way out of the promotion, including, but not limited to, where arising out of the following: (a) any technical difficulties or equipment malfunction (whether or not under the Promoter’s control); (b) any theft, unauthorised access or third party interference; (c) any entry or prize claim that is late, lost, altered, damaged or misdirected (whether or not after their receipt by the Promoter) due to any reason beyond the reasonable control of the Promoter; (d) any variation in prize value to that stated in these Conditions of Entry; (e) any tax liability incurred by a winner or entrant or child; (f) any inability to take the prize as offered; and (g) use of the prize.
- As a condition of taking the prize, the winner and/or parent or guardian of the winning child and the authorized representative on behalf of the winning school must sign any legal documentation as and in the form required by the Promoter in their absolute discretion including but not limited to a legal release and indemnity form.
- The Promoter collects entrants’, parents’, guardians’, Childhood Heroes’ and authorised representatives’ of schools (hereafter collectively referred to as “individuals”) personal information in order to conduct the promotion. If the information requested is not provided, the individual (or school, when applicable) may not participate in the promotion, or disqualify an individual from entering. The personal information may be collected by third parties, such as, FUJIFILM Australia Pty Ltd (“Fujifilm”) for and on behalf of the Promoter, for the purpose of conducting the competition.
